Step 1

To do this, we need to consider the unique aspects of each participant and the additional information provided. Participant A has a serum folate level of 11-14 nmol/L. This is within the normal range, so it is unlikely that this participant would display abnormal  Show more…

A 22-year-old Caucasian woman of Italian heritage had been told that she was slightly anemic and had been treated with iron periodically throughout her life. She was a student in a clinical laboratory science program and had a CBC performed as a part of a hematology class. Laboratory values are as follows: REFERENCE RANGE Hemoglobin 11.0 g/dL 11.7-15.7 g/dL Hematocrit 34% 35-47% RBC 5.8 x 10^12/L 3.8-5.2 x 10^12/L MCV 59.4 fL 80-100 fL MCHC 31.9% 32-36% RDW 14.2% 11.5-14.5% From these values, the hematology instructor suspected an inherited disorder instead of iron deficiency and suggested that the student contact her physician for further testing. A hemoglobin electrophoresis revealed slightly increased amounts of hemoglobins F and A2, which were subsequently quantitated to reveal hemoglobin F of 3.2% and hemoglobin A2 of 4.2%. Iron studies were normal. QUESTIONS: 1. What is the most probable disorder? 2. What CBC values caused the instructor to suggest further testing? 3. Why is it important for this disorder to be correctly diagnosed?

Calculate MCV, MCH, MCHC. Indicate which results are abnormal, stating whether they are low, high, or potentially critical. Predict what kind of anemia the patient might have based on their interpretations.
