Admitting Diagnosis: Clotted AV graft Primary Procedure: thrombectomy and angioplasty DESCRIPTION OF PROCEDURE Preop Diagnosis: Clotted left forearm dialysis AV graft, ESRD Postop Diagnosis: same Surgeon: Dr. Charlotte Lee M.D. Indication: During dialysis, pain, redness and swelling of the AV graft site was noted with decreased blood flow. Thrombectomy and angioplasty are planned to restore blood flow to the graft. Description of Procedure: Patient was transported to the catheter lab and prepped. Using sterile technique, ultrasound guidance and a micropuncture set, access was obtained to the arterial side of the graft and a 7-french sheath was placed with the tip directed toward the venous anastomosis. A 7 mm balloon was then advanced across the venous anastomosis and multiple dilations were performed. The thrombus was removed from the graft using the clot-buster device. Heparin was injected into the graft. An additional 5french sheath was then placed in the venous side of the graft with the tip directed toward the arterial anastomosis. A 5-French Fogarty catheter was advanced across the arterial anastomosis and pulled back into the graft. The pulled thrombus was removed. Additional dilations were then performed on the venous side of the graft and across the venous anastomosis. The graft was checked for patency. Good blood flow was confirmed. The patient tolerated the procedure well and was transported to post op observation.
