Case 1: Upon presentation: An 18-month-old female arrives by ambulance at the emergency department. The paramedics report that there was no known history of any recent trauma, and no known fever, vomiting, or other unusual behavior. There were no known ingestions or medications in the household. There was no evidence of trauma. Interview and History: At 9 PM the previous night, Ella was described by her mother as appearing more quiet than usual. They had spent the day traveling from the grandmother's house and Ella had been carsick so had not eaten very much during the day. When they got home, Ella had some water and went to bed. Ella slept longer than usual and was found unresponsive by her mother at 9AM; at this time her mother called 911. Follow-up tests: 1. Blood glucose: 23 mg/dL (normal range 90 – 125 mg/dL) 2. 48-year-old registered nurse was taking routine care of patient when despite years of training and experience, an IV needle accidentally elongated her arm and jabbed into her skin. Needlestick injuries are an occupational hazard in the medical field, with some force into and 800,000 needlesticks occur every year. Exposure to HIV via this route has the potential to cause infections in every 1000 cases. Antiseptic infections are high enough risk to warrant post-exposure prophylaxis. In this case, the nurse immediately washed the wound, applied betadine antiseptic to the area, and reported the injury to her supervisor. Unfortunately, the patient she was caring for was HIV positive, which necessitated that she immediately began a course of several anti-HIV drugs for post-exposure prophylaxis. Unfortunately, within a few days of the injury, the patient she was treating died of Pneumocystis pneumonia. The nurse discontinued the anti-HIV drug regimen after two weeks due to severe side effects, hoping that the two-week course prevented the infection. However, her condition worsened and the area around the injury became red and swollen, and she developed a 102°F fever. Routine biopsies were clear of any bacterial or fungal cause of infection, but she was prescribed a beta-lactam antibiotic anyway. The nurse became alarmed when the infection spread to a lymph node in her armpit, and routine screening for tuberculosis showed a positive result for TB exposure. The abscess on her arm was re-biopsied and showed the presence of Mycobacterium tuberculosis. It was surmised that the patient who died from Pneumocystis pneumonia was also infected with Mycobacterium tuberculosis and the Pneumocystis masked the secondary infection. The nurse immediately began a multi-drug course of antibiotics to treat the abscess caused by tuberculosis. Following this six-month course of treatment, the abscess healed and HIV tests came back negative. Multi-drug therapy was important for both types of infection in this case, HIV and tuberculosis, as both were effectively treated when multiple drugs were used.
