HEPARIN DRIP CALCULATIONS: Calculate rate and units/hour using:
Heparin drip using:
1. 25,000 units/500cc D5W
2. Heparin 25,000 units/250 cc D5W
1. Patient weight: 85 kg. Order: 4000 unit bolus of Heparin with a continuous Heparin drip at 1000 units/hour.
a. PTT = 36. Give 40 units/kg bolus. Increase drip by 2 units/kg/hour.
2. Patient weight: 100 kg. Order: 7000 unit bolus of Heparin with a continuous Heparin drip at 1400 units/hour.
a. PTT = 80. Decrease drip rate by 2 units/kg/hour.
3. Patient weight: 92 kg. Order: 5000 unit bolus of Heparin with a continuous Heparin drip at 900 units/hour.
a. PTT < 33. 80 units/kg bolus. Increase drip rate by 4 units/kg/hour.
4. Patient weight: 60 kg. Order: 4000 unit bolus of Heparin with a continuous Heparin drip at 1100 units/hour.
a. PTT = 50. Give 40 units/kg bolus. Increase drip by 2 units/kg/hour.
