Text: HP2020 Impact Paper: Disability and Health, state - Illinois Identify your target population (for instance, age or other demographic, aggregate population); this must be in your city or state. Discuss population-based health education interventions for your target population that are aimed at reducing morbidity and mortality for the problem. Be sure to review the research literature and HP2020 for interventions. Identify how and what data for interventions are being tracked. In a four (4) page paper, address the following. Identify the problem in the introduction section. Provide an overview of the problem in your state/national. Review descriptive epidemiological and demographic data on mortality/morbidity and risk. The organization is interested in establishing a community outreach program promoting smoking cessation and lung cancer screening. The Board of Directors requested your services in processing the data collected through the prospective cohort study among the male and female population visiting the local gym. a. The dataset is attached. Please use Excel or Epi Info to process the data. Choose the correct epidemiological tool, provide a graphic representation of the processed dataset, and offer a conclusion as to the causative relationship between smoking and death. b.
