A 65-year-old male was in the Intensive Care Unit after suffering a massive heart attack. Three days before his discharge, the nurse on duty noticed that the patient had a slightly elevated temperature of 100.4 degrees Fahrenheit. The doctor on duty gave Tylenol. The fever was reduced. The next day, the patient complained of shortness of breath, cough, and chest pain. The EKG was normal, but the lungs were filling with fluid and his fever returned. The patient noticed that his night duty nurse 3 days ago was coughing and had a runny nose. When she was done taking his vitals, she did not wash or sanitize her hands upon leaving his room. A CBC (Complete Blood Count) and CMP (Comprehensive Metabolic Panel) were also drawn; WBC was 11,400/mL and BUN (Blood Urea Nitrogen), Creatinine Clearance, and Kidney Functions were all within the normal range. After ruling out another cardiac episode, the doctor ordered a throat culture and a sputum culture. The throat culture was negative, but the sputum culture showed gram-negative coccobacilli growth on TSA plates. On MacConkey agar, the microbe was shown as yellow on the plate. Broad-spectrum antibiotics were started while the results of the cultures were still growing, but they didn't help. Once the results of the culture were back, the doctor started the patient on a cocktail of imipenem, cilastatin, and relebactam (Brand Name – Recarbrio). The patient was feeling better within 14 days of treatment and was discharged once 2 consecutive sputum cultures were collected with no growth. What microorganism was infecting the patient? Lab finding: How would you diagnose this organism? What should you find?
