Case: Maci Maci is 57 years old and retired after working 35 years in a textile mill in Southeast Alabama. Maci and Weldon, her husband of 37 years, decided to move back home to Memphis to be closer to their daughter's family and their grandchildren. Maci and Weldon love to be around their grandkids, but they run out of energy fast. Several years ago, Maci was diagnosed with osteoarthritis (OA) in both knees and her low back. Two years ago, she experienced unusual shortness of breath (SOB) while working her regular shift at the mill. The company physician diagnosed her with emphysema and last year, with the help of her physician and Chantix, she finally quit her 1 ½ pack/day cigarette habit. Sometimes, when running errands and visiting her grandkids, she needs supplemental oxygen to maintain her oxyhemoglobin saturation and reduce the frequency and duration of her bouts with shortness of breath. Maci wants more than anything to gain more stamina, so she can enjoy being with her family. She also wants to see her grandkids grow up. She's tried to walk with Weldon, but he has a regular routine of brisk walking. It is hard for Maci to keep up because of her OA and occasional SOB. You are working at a hospital-based wellness center where Maci recently had a check-up. Her physician made sure her Toprol XL and Celebrex were being taken as prescribed and was working. cleared her to begin a program monitored physical activity in your wellness center. Maci agreed to share with you the following information from her most recent checkup: Height = 64" HR resting = 88 bpm Cholesterol = 134 mg/dL TG = 140 mg/dL Body Weight = 121 lbs SBP resting = 132 mmHg eGFR = 52 ml/min/1.73 m² Blood Glucose fasting = 85 mg/dL Waist = 30 in DBP resting = 82 mmHg HDL-C = 68 mg/dL HbA1C = 5.8% Case Study Issues and Questions Pre-test observations: This is the process of organizing Maci's information so that you can safely prepare her for physical assessments before recommending any type of physical activity. 1. Provide a thorough summary of Maci's health information that was collected during his physical exam with her doctor. Do not restate the results. Summarize the findings, calculate important health parameters, compare her results to norm-reference standards and guidelines found in GETP10. Include insights from what you observe from her background information and check-up. (These observations will help Maci and your team understand the primary and secondary goals for her physical activity program.) 2. What additional health-related information would you like to know about Maci prior to preparing her to begin physical activity?

A 58 year old woman who experienced her first MI and is being discharged home. She currently works full time and is divorced. She lives in an apartment and has no family in the surrounding community. Be prepared to discuss the following: What should be the focus for her nutritional history and assessment? What dietary recommendations should be made? What obstacles to staying on the diet recommended might this woman encounter? What special considerations should you, as a nurse, be aware of?
