You are performing vital signs and an initial intake assessment on an adolescent mother who is bringing her 3-month-old infant for her first set of immunizations (the child had received the first hepatitis vaccine after birth). You weigh the infant, measure length and head circumference, and plot your findings on a national growth chart. You become alarmed that the infant is demonstrating a third percentile for each of the three measurements. You orient the family to the room and tell them you will be back to provide care.
