WEEK 2 discussion: Mono Nu 44-year-old Filipino patient This week case study is about a patient who is 44-year-old and on blood thinner. When this patient present to the clinic we must be careful not to be led by some bias as it mostly happens to people who are not from the united but be culturally educated and pay attention to the socioeconomic, spiritual, lifestyle and other cultural factors which might be affecting this patient understanding and behavior. These behaviors might be the cause of the patient statement about the blood thinner not working. We have a clue as it was mentioned in the scenario that patient is on a special diet which is comprise of Tofu and Fish. Patient diet might be an issue here because even though fish and Tofu are healthy diet and could help with high blood pressure, they also contain high dose of Omega 3 fatty acids which can altered the blood thinner action in the body causing it not to work. The second though about patient medication not working might be their lifestyle. Patient stated taking medications as prescribed, but we are not sure whether he has been busy working and not t asking them at the schedule time or was too busy to rest as they suppose to. Rest is a big part of healing, not resting enough might make patient labs still off and showing that the medication is not working. We also must assess patient personal belief and find out if there is anything in the spiritual background that will affect their acceptance of the treatment and move quickly towards healing. We must be sensitive to patient culture and try to understand what might possibly be hindering their treatment, if the have some cultural belief that might be affecting their compliance with the medication. We will have to pause and take some time to learn from patient about their cultural background. We will need to understand patient vision and behavior towards taking a blood
thinner. If there is any person or family member who is opposing the regimen he is on, if his lifestyle stays the same, we need to come into an agreement for a moment, because the treatment will not work no manner what we do. Socially and culturally people from Filipino have a strong belief in family life, and a spiritual foundation with a serious understanding of faith. We will need to get to their level and understand what they are filling about their prescription, lifestyle change, element in their culture that the believe might help instead of the medication, any other socio-cultural practice they are observing in association with the blood thinner prescription. All these concerns need to be assessed and work on by the nurse practitioner before moving forward with the blood thinner regimen.
