The Medicare Hospital Readmissions Reduc on Program (HRRP) was implemented by Centers for
Medicare and Medicaid Services in 2o12 to reduce the rate of readmissions in hospitals. Unplanned
readmissions have been associated with lower quality of care and costly for the healthcare system and the pa ent. The purpose of the program is to reduce this rate by penalizing hospitals for excess
strategies, and strategies to target social determinants of health in reducing unplanned readmissions (Joshi et al., 2015). A gap would exist if a pa ent con nuously were being readmi ed to the hospital because it shows that the pa ent's diagnosis is not being resolved and the proper instruc ons a er discharge are not being provided.
If the Quadruple Aim framework is applied to the HRRP, it reduces costs for payers, rewards hospitals for high-quality care, reduces readmissions, and provides be er care overall for the pa ent.
