The 12 Core Functions
The 12 core functions of substance abuse and treatment are a set of blueprints that are
used by substance abuse counselors to effectively diagnose and treat their clients. These
functions can help a substance abuse counselor get to know their client and their substance abuse
issue. In our case study of Joe Smith, we used the 12 core functions to diagnose and create a
treatment plan that fit his needs. Screening was used to gather information to determine if Joe
Smith was a viable candidate for treatment. We were not involved in Joe Smith's intake and
orientation however it was determined that Joe did qualify and could benefit from a residential
treatment program. While we did not perform Joe Smith's initial assessment, we were provided
with a detailed report of his initial assessment, and this is the information that we used in our
screening process, diagnosis, and development of his treatment plan. We did develop an
appropriate treatment plan for Joe smith based on the information that we learned from the
assessment report. We were not involved in Joe Smith's counseling, case management, or crisis
intervention, but we were able to refer him for residential treatment and counseling. We were
able to provide some client education, referrals, consultation with other professionals, and
reports and record keeping through our treatment plan and SMART goals.
Not being able to meet with Joe Smith in person did limit us. Because of this we were not
able to assess Joe's body language or see if he was in active withdrawal during any of his
assessments and meetings. Also not being able to meet with Joe face to face limited us on which
of the 12 core functions we would be a part of as well. We were not part of Joe's intake,
orientation, counseling, case management, or crisis intervention.

Learning about the 12 core functions has helped me to better understand the diagnosis
and treatment plan process. It has helped me to familiarize myself with what goes into
assessment and screening. Prior to learning about the 12 core functions, I did not know that
assessment and screening were considered separate from one another. I also thought that case
management would automatically include things like crisis intervention, client education.
referrals, and record keeping.
Gathering Client Information
The limitations of using the biopsychosocial approach in gathering information from Joe
Smith is that we were given set data. This limited us because we could not dive deeper and
question Joe further when there were things that needed to be clarified. For example, when we
had to arrive at a diagnosis using the DSM-worksheet there were things that should have been
