Nursing Skill - Percutaneous Transluminal Angioplasty

ACTIVE LEARNING TEMPLATE: Nursing Skill STUDENT NAME SKILL NAME Percutaneous transluminal angioplasty REVIEW MODULE CHAPTER Description of Skill Percutaneous transluminal angioplasty is an invasive intra-arterial procedure using a balloon and stent to open and help maintain the patency of the vessel. Indications CONSIDERATIONS -can be performed on an elective basis to Nursing Interventions (pre, intra, post) an occlusion of one or two coronary arteries. -might reduce ischemia during the occurance of and acute MI. performed withing 4-6 hours of the onset of manifestations if having an NSTEMI, 60-90 minutes for a STEMI. -could be used as an alternative to CABG -obtain infromed consent -monitor patiet for arrhythmias and chest pain after procedure -obtain vital signs -administer sedatives and analgesia as prescribed -assess the affected extremity for bleeding. or hematoma at insertion site. Outcomes/Evaluation Client Education -relief of chest pain -pressure will be applied to the insertion site -keep leg straingt -report chest pain, shortness of breath, and cardiac manifestations -teach about medications prescribed -teach patient how to reduce risks for CAD -artery is no longer occluded Potential Complications Artery dissection (perforation of an artery by the catheter) Cardiac Tamponade Hematoma formation near insertion site allergic reaction related to the contrast dye Embolism retroperitoneal bleeding Restenosis of treated vessel Nursing Interventions -notify provider immediately -administer IV fluids to manage hypotension assess insertion site for development of hematoma hold pressure for uncontrolled oozing/ bleeding -monitor for allergic reaction -monitor vital signs and saO2 -assess for flank pain and hypotension ACTIVE LEARNING TEMPLATES THERAPEUTIC PROCEDURE A9
