PURPOSE OF MEDICATION

Medication binds to the cell wall of the bacteria destroying it and killing the cell
Kills gram-positive pathogens such as staphylococci, group a beta hemolytic streptococci, streptococci pneumonie
Complications
Medication Administration Oral Vanco myosin given for staphylococcal enterocolitis and C Diff only IV intermittent dilute reconstitute 500 mg vile with 10 ML sterile water infused over one hour don't infuse rapidly or as a bolus
Redman syndrome, phlebitis at site, auto toxicity, rash, hypotension, nausea, vomiting, nephrotoxicity, leukopenia, eosinophilia
Contraindications/Precautions Hypersensitivity to vancomycin, renal impairment, hearing impairment, intestinal obstruction or information, OB
Nursing Interventions
So signs of infection, assess IV site for your rotation, monitor blood pressure, monitor I & Os, Monitor appearance of urine, evaluate eighth cranial nerve, observe for signs of super infection, assess bowel sounds
Interactions
Aspirin, aminoglycosides, cyclosporine, cisplatin, loop diuretics cause an additive effects to auto toxicity and nephrotoxicity. Nondepolarizing neuromuscular blocking agents, children experience histamine flush with general anesthetic
Client Education Do not double dose, report any signs of allergic reaction, notify HCP if no signs of improvement in a few days, patient with history of rheumatic heart disease should take for any dental procedure
Evaluation of Medication Effectiveness The patient no longer shows signs of infection wound is afebrile
