Description of Skill
A new guaze dressing will be placed on patient wound
Indications To promote wound healing, removal of exudate, necrotic debris, and bacterial contaminants
CONSIDERATIONS
Nursing Interventions (pre, intra, post) Pre: Nurse will check physicians order, prepare materials for dressing change, and assess the dressings condition Intra: Nurse will ensure hand hygiene and remove old dressing with gloves. -Assess wound and clean wound using aseptic technique and place a new dressing. Post: Assess patient for bleeding and pain and educate patient on a follow up.
Outcomes/Evaluation Wound healing Pain control - No signs and symptoms of infection Skin integrity is imporved - Improved mobility
Client Education Educate Patient the proper steps of wound care: - Avoid getting wound wet with non sterile water. - Educate patient about nutritional managment - Educate about pain medication Explain follow up of care and report any abnormal discharge from the wound site.
