BCR-ABL Fusion Gene

The BCR-ABL fusion gene is created by a chromosomal translocation that fuses parts of two separate genes. Although it produces an abnormal, constitutively active tyrosine kinase that can drive cellular proliferation, its presence alone does not guarantee that cancer will develop.

Cancer development is typically a multistep process requiring several genetic or epigenetic changes. The presence of the BCR-ABL fusion gene represents one event, but additional mutations or alterations in cellular regulation are usually needed for full transformation into malignancy.

Preleukemic Clones and Clonal Hematopoiesis

Low-level clonal expansions or preleukemic clones can exist in healthy individuals. These cells may carry oncogenic mutations like the BCR-ABL fusion without progressing to leukemia due to the absence of further genetic hits and because regulatory mechanisms prevent their uncontrolled growth.

Gene Expression Thresholds and Regulatory Mechanisms

Detection of transcript does not always equate to the level of active protein necessary to disrupt normal cellular functions. Cellular control mechanisms such as apoptosis, cell cycle regulation, and immune surveillance can restrict the impact of low-level oncogene expression, preventing the onset of diseases like chronic myelogenous leukemia.
