Case Study Discussion – Part II:

Reflecting back on your previous post, how successful were you in identifying the possible diagnosis? What aspects from the case study were the most relevant in order to help confirm this diagnosis?
Formulate your plan of care for the principal diagnosis by addressing each component listed below:
Pharmacologic Treatment Plan. The plan includes ALL of the following: drug name, dose, route, frequency, duration, and cost as well as education related to the pharmacologic agent. If the diagnosis is a chronic problem, the student includes instructions on currently prescribed medications as above. Cite each medication with the prescribing reference that was used to justify and verify an evidence-based pharmacologic treatment plan. This can be an application such as Epocrates, Medscape, etc. Justify the treatment plan according to clinical practice guidelines, meta-analyses, peer-reviewed studies, regional antibiograms, etc.
Non-Pharmacologic Treatment Plan. Includes a detailed non-pharmacologic treatment plan for the principal diagnosis with a minimum of three recommendations for the principal diagnosis. Provide rationale and cite each statement with evidence-based research to justify your non-pharmacologic treatment plan.
Non-Pharmacologic Treatment Plan (Diagnostic Testing). Includes appropriate diagnostic testing 100% of the time OR acknowledges “no diagnostic testing clinically required at this time”. Provide a rationale for each diagnostic test with consideration of cost and if preauthorization is required. Cite each statement with evidence-based research to justify your non-pharmacologic treatment plan.
Anticipatory Guidelines. Includes at least 5 primary and secondary prevention strategies, using the AHRQ ePSS app, based on the patient’s age, sex, pregnancy or immunization status, tobacco use, and/or sexual activity. Must include at least 3 secondary prevention strategies.
Follow-up plan. Includes recommendation for follow-up, including time frame (i.e. x # of days/weeks/months), referrals, and red flags. Must include evidence-based research citations to support decision-making.
*Each component of your plan should include rationale and evidence-based research citations. Include at least two peer-reviewed journals.
