Jane is a 14-year-old female who was referred to her primary care physician for counseling on weight management. She is consistently gained 10 to 15 pounds each year for the last six yearsand has progressed from the overweight BMI category into the obese BMI category for her age. She is currently 62 inches tall or 5 feet 2 inches she weighs 152.9 pounds and has a BMI of 27.8this BMI category categorizes her as obese and places her into the 95th percentile for her gender and age. Jane lives at home with her parents and two younger siblings both of her parents have reported struggling with their weight themselves. Her mother is currently obese and has been diagnosed with hyperlipidemia. Her father also obese has a diagnosis of type 2 diabetes mellitus. Her family also reported limited income and often runs out of money before the end of the month budgeting can be challenging for a family of five. So, they often rely on school meals and fast food to save money throughout the week. The family also reported no physical activity. In Jane's eating habits and diet preferences are highly influenced by her parents. At Jane's most recent physical exam, her primary care physician did a full nutrition assessment including labs. While most Labs came back within normal limits, her fasting glucose, HDL cholesterol and LDL cholesterol all fell outside the normal range indicating the need for immediate intervention andlifestyle changes. At this session Jane also reported a 24-hour dietary recall which indicated that she is consuming excessive energy intakes primarily choosing refined carbohydrates, saturated fats, processed proteins, excessive sodium intakes.
