A 77-year-old male client with a history of chronic obstructive pulmonary disease was diagnosed with benign prostatic hyperplasia and prescribed terazosin. He presented to the emergency department 2 days ago with acute mental confusion and was diagnosed with acute urinary retention, urinary tract infection, hematuria, and hydronephrosis. The client underwent a transurethral resection of the prostate (TURP) procedure and has been transferred to the medical-surgical unit.
For each client statement, check the box to indicate whether the nurse’s discharge instructions were Effective (helped the client understand the discharge instructions), Ineffective (did not help the client understand the discharge instructions), or Unrelated (not related to the discharge teaching). Only select one indicator per statement.
I may still experience overflow incontinence due to ongoing urinary retention.
I will follow up with primary health care provider if I experience erectile dsyfx
I will drink at least 2000 mL of water daily to keep hydrated and my urine color clear
I may have temporary dribbling of urine and will wear an incontinence pad until I am confident that this has resolved
I plan to walk for 30 min each day and rest if I experience pain or SOB
