The family member states that the client lives alone at home. "I check in every so often. When I checked this morning, they
said they did not feel well last week and did not go to dialysis."
Upon assessment, the client is alert and oriented to person and place only. They have crackles noted bilaterally, S1 and S2
noted, with bilateral upper and lower extremity swelling 3+. Pulses are 2+ throughout, the capillary refill is less than 3 sec,
and the skin is dry and flaky. The client has an ammonia odor to their breath and reports itchy skin. The client is anuric.
The last bowel movement was yesterday.
