Mrs Chen is 78-year-old lady from China, living in Inglewood, Regional Queensland. Mrs Chen is diagnosed with stage 5 Chronic Kidney Disease (known as end-stage kidney disease or ESKD) and is undergoing dialysis every 3 days. She in attendance today, and you are nurse looking after her. She attends Toowoomba hospital (nearest hospital with dialysis services), which is 150 km away from Inglewood, and it takes 1 hr and 45-minute drive to reach Toowoomba hospital. Mrs Chen is also diagnosed with Dementia, Stage 4 on Global Deterioration Scale (GDS). Mrs Chen lives with her husband Mr. Chen, who is 76, in good health and mobilizing regularly. Mrs Chen or one of their children is driving Mrs Chen to Toowoomba for dialysis. Mr. Chen still works in the family-owned Food takeaway shop, decreased hours. Mrs. and Mr. Chen have 3 adult children, leaving nearby, 2 out of 3 working in the shop as well. The family is generally healthy, eating culturally prepared food and like to practice tai chi, however, Mrs. Chen is not very active lately as she states she feels very tired. Family has noticed that Mrs Chen is having short memory loss very often, and sometimes does not remember specific events related to her. Mrs Chen needs constant reminders to take her regular medication, however, the family is noticed that she becomes anxious when she needs to travel to Toowoomba and try to avoid it if possible. Mrs Chen speaks English well, however, recently family has noticed that she is forgetting words, and speaking more Chinese than English. Mrs. Chen also has hypertension, controlled by perindopril and dapagliflozin to reduce risk of progressive decline, however, often Mrs Chen forgets to take her medication. 1. Identify and briefly describe current chronic condition/s associated with the person in the selected case study. Analyse the impact of the chronic condition/s on a person’s well-being. 2. Identify and describe the determinants of health that may impact self-management of the identified condition for the person in the selected study. 3. Critically discuss how identified chronic disease and the determinants of health can cause deterioration of the overall health and well-being of the person in the selected case study. Deterioration should be clearly identified, with examples provided. 4. Discuss and justify one (1) primary, and one (1) tertiary health promotion strategy to support self-management for the person in the selected case study.
