Date: June 15, 2016, Mr. Smith is a Resident on your assignment. The nurse has informed you that his physician visited at 0600 this am and has placed him on Intake and output for the next 24 hours. Record the following information appropriately on the In and Out sheet.
At 0717 he had 2/3 of a lg. drinking glass of water. At 0740 he voided 300 ml. he went to breakfast at 0800 and had 1 cup of coffee with 2 creamers, ¾ of a small glass of orange juice, 2 slices of toast, a bowl of cereal and one carton of milk. At 0845 he voided 230 ml.
At 1045 he had a pot of tea, with 3 creamers, and 3 apple slices with 2 pieces of cheese. He had a nap prior to lunch.
He was up at 1145 and voided 245 ml. and at 1215 he had lunch consisting of 1 cup of coffee with 2 creamers, 2/3 of a bowl of soup, 1 pc. of lasagna with 2 pc. of garlic toast, 1 (1)/(2) lg. drinking glasses of water and 1 carton of milk. For dessert he had a bowl of jello with a creamer.
Mr. Smith went on a bus tour for 3 hours in the afternoon. At 1610 he voided 310 ml. During supper, at 1630, Mr. Smith consumed 2/3 of a small cup of soup, 1 small glass of pear juice, ¾ of a carton of milk, 1 egg salad sandwich, 5 carrot sticks, and a small garden salad. For dessert he had an ice cream cup with banana slices.
At 1830 he had ¼ sm. drinking glass of water. At 1845 he voided 280 ml.
At 2015 he had a cup of tea with 1 creamer, and 2 sugars, and 3 digestive cookies. Just prior to bed, at 2130 he voided 285 ml.
At 2245 Mr. Smith was up and vomited 330 ml. At 2250 he was given a small juice glass of ginger ale, he drank 1/3 of it and immediately after vomited another 190 ml. He was given a Gravol suppository, and settled to bed. At 2330 he had (1)/(2) sm. glass of water.
At 0230 he was up to the bathroom and had 275 ml. of diarrhea, at 0245 he was up again and voided 110 ml. he drank (1)/(2) sm. drinking glass of water. At 0255 he vomited 210 ml. then settled back to bed.
At 0550 he had ¾ large glass of apple juice, and was up to void 90 ml. at 0605. At 0645 he was again up to void 230 ml. and at 0650 drank ¾ small juice glass of Ginger ale, then settled back to bed.
