Peer response to One clinical client-focused quality measure currently analyzed in the ER I practice in is time for antibiotic administration for sepsis or SIRS patients. The data collected for patients suspected of being septic or in SIRS consist of the time the sepsis alert is called, time the IV fluid resuscitation, labs, and when antibiotics are ordered and administered. The type of antibiotics used can also be collected. In my practice area, we have papers used to guide you, avoid falling out of protocol, and potentially compromising the patient. The data collected can be primarily found in the electronic health records (EHRs). TImes of medication administration and orders of the treatment decided by the physician can be found in the EHR. The persons in charge of collecting the data include nurses and physicians, however, anyone can refer to the EHR to obtain data. Analysis is done by those that work in clinical informatics and quality analysts who are responsible for following up on treatment delays and the reason for those delays. Delay in treatment can make a massive difference in mortality of these patients. Data collected undergoes thorough evaluation by contrasting it with different benchmarks for sepsis. Analysts could also investigate trends within multiple sepsis patients and attempt to identify any other issues that may be systemic and the cause of treatment delay. Advanced practice nurses (APNs) and physicians from different interdisciplinary teams make decisions to apply the data to practice change. A key challenge when evaluating quality measures in healthcare settings could be the timely administration of antibiotics. It is crucial to administer them, but becomes conflicting when considering the potential risks of antibiotic resistance and increasing the prevalence and rate of C.diff.
