Mr. George Moore, a 78-year-old African American client was brought to the emergency room by ambulance following a motor vehicle accident.
The client was found unconscious at the scene. He is now awake with complaints of left hip pain, left arm pain, and a large bleeding laceration on the left side of his forehead. His past medical history includes hypertension, diabetes, and seizures. He has blurred vision at the present time. On assessment, the nurse documents PERRLA. His weight is 210 pounds. NKA. Vital signs are: T=99.0°F (37.2°C), P=100 and regular rhythm, B/P=180/90, SaO2=94% RA, pain level 7/10.
