Cardiovascular Case Study Patient: 56-year-old airline bank manager Signs & Symptoms: • severe, intense pain radiating to left shoulder & down the left arm • precordial, crushing sensation radiating to left shoulder & down the inside of the left arm due to a tennis game The discomfort on the chest was relieved when the patient rested. Examination at the emergency room revealed the following: Heart rate (HR) 96 b/min Blood pressure (BP) 156/112 mm Hg ECG: ventricular extra-systole arrhythmia (premature ventricular contraction [PVC]) as well as S-T segment depression and decreased R wave height. An exercise tolerance test showed an ischemic ECG when the patient was exercising revealing a downward-sloping S-T segment. In addition, chest pain was felt with mild exercise but relieved with sublingual nitroglycerin. There was obstruction of the coronary arteries at >75%. The pharmacologic therapy included beta-blockers, calcium channel blockers, & nitroglycerin. The patient underwent an angioplasty where the coronary flow returned to normal due to an inserted balloon-tipped catheter into the obstructed vessels. A.
