A 20 year old woman complained of a worsening headache and felt feverish. In the following 2 hours, she displayed irritability and a decreased ability to concentrate. She became drowsy and it became increasingly difficult to arouse her to a conscious state. An hour later, the emergency room performed a lumbar puncture (spinal tap). The cerebral spinal fluid appeared cloudy and was sent to the microbiology lab.

A 20-year-old woman arrived at an immediate-care medical facility with recurring fever and chills. She reported an episode that began about 30 minutes before her admission. She had headaches, muscle aches, and nausea for 4 days before the beginning of the fever and chills. She told the physician that she had recently returned from a missionary trip to Honduras and thought she might have the flu. A complete blood count and urinalysis were ordered but the results were unremarkable. The differential report, however, noted the parasite forms seen in the accompanying figure.

A 78-year-old woman developed a rapid onset of fever and a shaking chill. She had difficulty breathing and produced thick (mucopurulent), rust-colored sputum. Shortly thereafter, she developed a headache, lethargy, and a stiff neck and lapsed into a coma. She was admitted to the hospital where blood, sputum, and cerebrospinal fluid specimens were obtained for culture. The laboratory shortly reported that Gram-stained smears of the sputum and cerebrospinal fluid revealed gram-positive, lancet-shaped diplococci. The patient was treated with intravenous antimicrobial agents, woke up from the coma, and recovered without incident. What was the likely identification of the patient's isolate? Select one: a. Streptococcus pneumoniae b. Escherichia coli c. Haemophilus influenzae d. Cryptococcus neoformans

A woman comes to the doctor with a two-week history of complete paralysis of her left arm. She has had no injury to the extremity, and full neurological workup fails to demonstrate any underlying cause. She seems surprisingly unconcerned about the paralysis, and seems more worried about an argument she had one month ago in which she hit her daughter. Based on this information, the woman's most likely diagnosis is: (A) $\quad$ conversion disorder. (B) $\quad$ generalized anxiety disorder (C) $\quad$ illness anxiety disorder (D) $\quad$ histrionic personality disorder.
