Signaling pathways can be altered by drugs in a variety of ways. Sildenafil (Viagra) is a phosphodiesterase type 5 inhibitor. How does sildenafil directly affect the nitric oxide pathway? It causes cGMP to lose a phosphate group. It causes phosphodiesterase to lock on, leading to widening of blood vessels. It causes blood vessels to widen by activating nitric oxide synthase (NOS). It phosphorylates acetylcholine. It allows cGMP to remain in the cell for a longer time, leading to prolonged action of the signaling pathway.
