An 18 y.o. woman came to her physician for a routine physical examination before entering college. Family history was unremarkable. Her personal history revealed that she had begun menstruation at the age of 13 and had experienced no complaints other than cramping. Other than an occasional cold, she stated that she was perfectly healthy. On physical examination, she appeared to be well nourished and looked her state age. Her overall muscle tone was poor but she explained that she "wasn't very athletic." Her pulse was 80 beat per minute, and her blood pressure was within normal limits. Results of laboratory testing included: WBC 11.4 x 10^9 /L Microcytes, some hypochromic cells, and polychromatophilic RBCs were seen on the peripheral blood smear. Hgb 8.0 g/dl MCV 79 fl RDW 14.8 PLT 400 x 10^9 /L. The reticulocyte count was 2.0% What is the most likely explanation of this presentation?

During his medical history, he states that he is active in sports and feels healthy. He is of Iranian descent; his family immigrated to the US 15 years ago from Iran. A CBC was ordered. Laboratory Data RBC 5.80 x 10^12/L Hb 10.8 g/dL Hct 37.1% RDW 13.6% PLT 326 x 10^9/L WBC 6.4 x 10^9/L Differential Segmented neutrophils 58 % Lymphocytes 33 % Monocytes 9% Questions 1. Calculate the RBC indices: MCV & MCHC. 2. Based on the RBC indices, what would you expect the RBC morphology (i.e., size and color) to be on the patient's Wright-stained peripheral blood smear? Continued Scenario Based on the CBC data, the physician ordered the following laboratory tests: Serum ferritin 155 ug/L (RI: 20-300 ug/L) Serum iron 102 ug/dL (RI: 65-180 ug/dL) TIBC 343 ug/dL (RI: 250-450 ug/dL) HbA2 5.9% Questions 3. What is the morphologic classification of this patient's anemia? 4. Based on this patient's laboratory findings, what is the most likely anemia? Be Specific. 5. Based on your response to Q #4, what results would you expect for the following new CBC and Reticulocyte parameters? Increased, Decreased, or Normal HYPO% MICRO% CHr

A 20-year-old woman comes to the physician because of an 8-year history of excessive bleeding with menses. Menses have occurred at regular 28-day intervals since menarche at the age of 12 years. She also has a history of easy bruising and frequent mild gingival bleeding after brushing her teeth. Her family history is unknown because she was adopted. Physical and pelvic examinations show no abnormalities. Laboratory studies show: Hemoglobin: 9 Hematocrit: 27% Leukocyte count: 5300 Platelet count: 145,000 Prothrombin time: 12 seconds (INR=1) Partial thromboplastin time: 48 seconds This patient's condition is most likely caused by a deficiency of which of the following factors? A) Epidermal growth factor B) Factor V Leiden C) Factor VII (proconvertin) D) Factor VIII (antihemophilic factor) E) Factor X (Stuart factor) F) von Willebrand factor
