Once upon a time there lived a man called Damocles. A friend of his eventually became the ruler of a small city. Damocles thought, ‘How lucky my friend is. He is now a ruler. He must be having a great time. He must have fine clothes, lots of money and a number of servants. I wish I had his luck.’ He decided to visit his friend to enjoy his hospitality. When he reached the palace, the king himself received him with respect and affection. Damocles then told the king that he was indeed a lucky man. The king smiled. He invited his friend to have dinner with him.

Situation: A 30-year-old male electrical technician was helping a company service representative test the voltage-regulating unit on a new rolling mill. While the electrical technician went to get the equipment service manual, the service representative opened the panel cover of the voltage regulator's control cabinet in preparation to trace the low-voltage (120 V) wiring in question (the wiring was not color-coded). The service representative was not using PPE. The service representative climbed onto a nearby cabinet to view the wires. The technician returned and began working inside the control cabinet, near exposed, energized electrical conductors. The technician tugged at the low-voltage wires while the service representative tried to identify them from above. Suddenly, the representative heard the victim making a gurgling sound and looked down to see the victim shaking as though he were being shocked. Cardiopulmonary resuscitation (CPR) was administered to the victim about 10 minutes later. He was pronounced dead almost 2 hours later as a result of his contact with an energized electrical conductor. Make a root cause analysis and list the procedures and steps that should have been implemented to prevent this accident.

One way to improve food security is to avoid food spoilage. Identify the correct ways that Devon reduced food spoilage in the passage. Devon prepared spaghetti and homemade sauce for dinner. The dinner served up to four people, but only Devon was around to eat it. He brought one serving to his neighbor's house and stored the other two servings in the refrigerator. Devon ate one more serving of spaghetti the following day. Later that week, his refrigerator broke, and the remaining spaghetti spoiled. He immediately called a repair service and got the refrigerator fixed.
