Upper vagina
Refer back to the Video Resource to study the ridged tube of the vagina and determine which portion is the upper vagina and which is the lower vagina based on the location where the vagina pierces through the pelvic diaphragm. In sagittal section, note the upper vagina's downwards and forwards inclination from the superiorly positioned vaginal vault to the inferiorly positioned vestibule.
The four vaginal fornices, at the most superior extent of the upper vagina, surround the ectocervix of the uterus to create a continuous moat-like ring-shaped depression anteriorly, posteriorly, and laterally (x2) vaginal fornices. The lumen of the vagina is generally collapsed anteriorly and posteriorly and is surrounded by muscular walls and closely anchored/related to the urethra anteriorly.
