A client has pain and decreased active ROM of bilateral shoulders, spine, and knees secondary to severe osteoarthritis. Symptoms interfere with the client's ability to independently complete lower body dressing. Which approach should the COTA use FIRST to improve the client's ability to self-dress? • Engage the client in a ROM and strengthening program to preserve joint integrity. • Identify the client's clothing preferences and typical dressing routine. • Select assistive devices to help improve functional performance. • Teach the client a variety of joint protection strategies for dressing.

CASE STUDY B: Rheumatoid Arthritis Ms. WP, a 42-year-old woman, has had rheumatoid arthritis for 6 years. She is experiencing an exacerbation of her symptoms. Her wrists are stiff, show slight ulnar deviation, and are red and swollen. Her fingers are also stiff and painful, and she is feeling extremely tired and depressed. Her elbows and knees are painful, especially after she has been resting. She has not been eating well. The reasons for the stiffness and deformity occurring at her wrists and fingers are due to the progression of rheumatoid arthritis. The factors contributing to the systemic symptoms noted in Ms. WP include inflammation and immune system dysfunction. To relieve the pain and stiffness, Ms. WP can try various treatments such as medication, physical therapy, and lifestyle changes. The activity of rheumatoid arthritis can be influenced by factors such as genetics, environmental triggers, and hormonal changes. In the treatment of rheumatoid arthritis, the following drugs act in different ways: 1. NSAIDs (Nonsteroidal Anti-Inflammatory Drugs) help reduce pain and inflammation. 2. Glucocorticoids (steroids) have anti-inflammatory effects and can suppress the immune system. 3. Disease-modifying agents slow down the progression of rheumatoid arthritis and help prevent joint damage. 4. Biologic agents target specific molecules involved in the immune response and can reduce inflammation. The possible course of this disease in Ms. WP will depend on various factors such as the effectiveness of treatment, adherence to medication, and the progression of the disease. Regular monitoring and management of symptoms will be important in controlling the disease and improving Ms. WP's quality of life.
