Jane is a 50-year-old woman with a 12-year history of severe alcoholism, with four hospital detoxifications and four chemical dependency treatments, none of which had any significant impact on her drinking. She was court-ordered to treatment for her third DUI (driving under the influence). After 2 weeks in the treatment center, the staff noted that Jane rarely was seen sleeping. At night, she would walk up and down the halls of the treatment center. She was also becoming more agitated in group therapy. In group therapy, she alternated between being very quiet yet agitated (unable to sit still) and very hostile when confronted or probed for information about herself by the counselor or other clients. She was the topic of most staff meetings, as counselors complained about how frustrating it was to work with her. There was discussion in the meetings of discharging her. The other clients increasingly refused to associate with her because of her highly critical manner and her tendency to be emotionally explosive. What aspects of this situation indicate that Jane may be struggling with a dual disorder? How would you initially intervene in the situation with Jane?
