Mrs. TT had a laboratory blood test done. Her test result shows that her thyroxine (T4) level is 7 picomol/L and thyroid-stimulating hormone (TSH) level is 8 mlU/L. What is a likely diagnosis for Mrs. TT based on her test result? Briefly explain your answer. T4 reference range: 10-25 picomol/L TSH reference range: 0.4-5 mlU/L

(A) Describe the hormone cascade pathway that controls the release of thyroid hormones. Make sure to include the 3 endocrine organs, the hormones they secrete, and the feedback regulation. (B) A lab test shows abnormally low levels of T4 hormone in your patient's blood. To diagnose the cause, the patient was given an injection of thyroid stimulating hormone (TSH), and then the thyroid hormone level was measured again. There was no change in the level of thyroid hormone in the patient. Which of the endocrine organs caused the abnormal T4 hormone levels? Explain with your answer in (A).
A twenty-four-year-old female presents for evaluation. She is a Pharmacology PhD candidate and she teaches at the nearby college. She is anxious by nature and appears to be always in a rush. Her appetite is normal. She has been feeling a bit run down, but assumed it was from lack of adequate sleep. She visits her PCP and he diagnoses hypothyroidism. Her TSH is assessed at 4.2. Thyroglobulin is slightly above the reference range. T4 is low which appears to be consistent with her condition. Physical palpation of her thyroid reveals a slightly enlarged gland with lumpy nodules. List at least four pertinent questions you would ask her to better collect data regarding her condition. 1. Have you noticed any changes in your weight recently? 2. Have you experienced any changes in your menstrual cycle? 3. Have you noticed any changes in your hair or skin? 4. Have you been experiencing any cold intolerance or sensitivity to cold temperatures? Would you order additional testing? If so, what specifically? Yes, I would order additional testing. Specifically, I would order a thyroid ultrasound to further evaluate the nodules and assess their characteristics. What is the likely diagnosis? The likely diagnosis is hypothyroidism. What medications would be appropriate for this abnormality? Levothyroxine, a synthetic thyroid hormone, would be appropriate for treating hypothyroidism. How might you carefully monitor this condition? This condition can be carefully monitored by regularly checking the patient's TSH levels and adjusting the dosage of levothyroxine accordingly. Regular physical examinations and thyroid ultrasounds may also be necessary to monitor the size and characteristics of the thyroid gland and nodules.
