Marcie is a new client and a 22-year-old female who works as a receptionist in a doctor's office. She has just revealed to you during the intake session that she has recently lost interest in most activities, has been sleeping a great deal yet feels tired all the time, and sometimes wishes she could cease to exist. She mentioned feeling as though she has been "on an emotional roller coaster" during the past year, throughout her on-again/off-again relationship with a 35-year-old married man. The last breakup with him seemed final, and Marcie has felt herself sinking deeper and deeper into depression ever since. When probed further about suicidal ideations, Marcie admitted that she has considered killing herself, although she is uncertain whether or not she would actually do it. She said that she is currently in possession of a gun that her friend allowed her to keep in her home following a rash of burglaries in the neighborhood, but she does not know whether she would actually use it. You have consulted with your supervisor, who has agreed that Marcie should be referred immediately for a psychiatric evaluation and has instructed you to arrange for Marcie to go directly from your office to a nearby hospital. Marcie told you that her mother accompanied her and is in the waiting room, but she has emphatically stated that she does not want her mother to know what is going on with her. Jean has suffered from depression his whole life. In college, he meets a woman, Katrinka, and falls in love. They talk about getting married. Katrinka is always talking about how they will love each other forever and grow old together. Jean cautions, "50% of marriages end in divorce, how do we know that won't be us? We could change. Maybe a prenuptial agreement would be a good idea in case that happens." Katrinka is hurt and outraged at the suggestion. What characteristic of depression is Jean demonstrating that Katrinka has difficulty appreciating?

Adriana is 26 and is studying at university. As a teenager she felt different and out of place among her peers as her family (who are from Italian descent and strict Roman Catholics) was very prescriptive of how Adriana could dress and behave. This is the first time Adriana has been living away from home and is finding the freedom liberating. In her first year at university, she develops a close friendship with one of her male lecturers. Adriana realises that she is attracted to him. The lecturer, Mark, is upfront with Adriana and tells her that he too is attracted to her but is married with two young children. Not long after, Mark and Adriana start a relationship. They keep this a secret because relationships between students and lecturers are not socially sanctioned and relationships where one person is married with children, are not always accepted. Adriana knows that her parents would be very upset by this development in her life. When Mark has a sudden heart attack and dies, Adriana is left bereft. She feels there is no one she can talk to about her loss and that she must hide what she is feeling because no one knows about her relationship with Mark. Five months after Mark’s death, Adriana has dropped out of university and is having difficulty maintaining daily activities. She prefers to spend most of her time alone in her room. Adriana is having difficulty sleeping and has lost a significant amount of weight. On the last counselling visit Adriana tells Judy, her support worker, that the whole world feels grey and she has panicky feelings and episodes where she cannot stop crying. answer the following: Describe the type of grief, and the reactions Adriana is displaying.
