Sarah had to go to a physical exam that would affect her insurance. She was 62 years of age, 190 lb., 5 foot 3 inches tall. Her blood pressure (BP) was 160/95 mmHg (average over 4 days). She was questioned and found to have mild polyuria. Her total peripheral resistance (TPR) was measured and found to be high. Her plasma renin activity was on the low end of the normal range. Her cardiac output (CO) was measured to be 6.8 L per minute. Imaging tests were performed. She was diagnosed as having hypertrophy of her left ventricle. Her eyes were examined, and retinal hemorrhages were found.
Problems for students to answer (by email or in this assignment):
1. Calculate the patient's presenting BP in terms of (a) pulse pressure (PP) and (b) mean arterial pressure (MABP).
2. Name FOUR specific lifestyle changes that should be recommended.
3. If these lifestyle changes alone do not change the disease significantly, what medications should be prescribed? (Name at least ONE initial drug and ONE additional drug if the initial drug doesn't sufficiently improve the patient)
4. What is an acceptable BP goal of treatment?
5. In addition to the two drugs you named in problem 3 (above), name a THIRD drug.
6. Describe how EACH of all THREE drugs work.
7. Describe what caused the hemorrhages of the patient's retinas.
8. Calculate the patient's GOAL BP in terms of (a) pulse pressure (PP) and (b) mean arterial pressure (MABP).
9. Compare the presenting PP and MABP to the goal PP and MABP.
