SCENARIO 1:
Baby Bayada was hospitalized from 7/29/20 to 7/30/20 for an Upper Respiratory Tract infection with a new weight of 34kg at discharge. 74.96 pounds He was prescribed 2 NEW medications while admitted.
Instructions:
You will transcribe these new meds from prescription in home using BAYADA PHARMACY, 770-914-0000, Prescription number 123-0006 to the attached addendum.
1. Albuterol 2.5mg/ml per vial (____ vial) nebulizer treatment every 4 hours PRN wheezing/O2 saturations below 92% on room air via nasal cannula.
2. Amoxicillin 200mg/5ml to administer 870mg (____ ml) BID FOR 5 DAYS via GT to start on 7/30/20.
3. Tylenol 160mg/5ml administer 510mg (____ ml) PRN every 6 hours for pain >3 on FLACC scale or fever greater than 100.3
