An 82-year-old female client was brought to the emergency department after tripping over a rug at home. The client is alert but in obvious pain. On assessment, the nurse notes the following:
The client reports severe pain in the left hip area, worsened by movement.
The left leg appears shorter than the right and is externally rotated.
Bruising is noted around the hip and thigh region.
Vital signs: BP 152/88, HR 110, RR 22, Temp 99.8°F (37.7°C), SpO₂ 94% on room air.
Past medical history: type 2 diabetes, hypertension, mild cognitive impairment.
The daughter is concerned about her mother’s ability to undergo surgery at her age.
Utilizing the clinical judgment model, answer the following questions:
1. Recognize Cues: Which findings are most concerning, and what might they suggest about the type of injury? What do the shortened, externally rotated leg and bruising indicate?
2. Analyze Cues: Based on the data, what possible conditions could explain this presentation? What complications are you most concerned about in the short term?
3. Prioritize Hypotheses: Which condition is most likely, given the combination of leg position, pain, and age? Which complications should be prioritized?
4. & 5 Generate Solutions and Take Action: What diagnostic tests or imaging would confirm your suspected condition? What preoperative interventions are appropriate while awaiting provider evaluation? Which immediate nursing actions should you initiate to stabilize the client? How should you involve the family in care planning at this stage?
6. Evaluate Outcomes: How will you know if your interventions (pain management, oxygen support, safety measures) are effective? What would indicate that the client is ready for the next step of care (surgery or alternative management)?
