SOAP Note
Subjective:
The patient presented with a history of abdominal pain (Hx) for the past week, worsening in intensity. The pain is described as sharp and localized to the right lower quadrant. No alleviating or aggravating factors were reported.
Objective:
During the physical exam (PE), tenderness was noted upon palpation of the right lower quadrant. The patient also had a low-grade fever of 100.4°F. Laboratory tests revealed an elevated white blood cell count.
Assessment:
Based on the history, physical exam findings, and test results, the patient is diagnosed (Dx) with acute appendicitis.
Plan:
The plan of care (Tx) includes immediate surgical intervention for an appendectomy to remove the inflamed appendix. Post-operative care will involve pain management and monitoring for any complications.
