Preoperative diagnosis: Complex congenital heart disease with ongoing need for vascular access Postoperative diagnosis: Complex congenital heart disease with ongoing need for vascular access Name of procedure: Placement of a Broviac catheter via left saphenous vein (Hint 2 codes) Description of procedure: With the patient in the supine position under general anesthesia, the lower abdomen, inguinal region and genitalia, and anterior left thigh were all prepped with Betadine and draped in a sterile fashion. We made a small transverse incision just below the inguinal crease and noted meticulous hemostasis as we dissected down onto the saphenous vein. The saphenous was isolated with 5-0 silk ties. We fashioned a subcutaneous tunnel from further down on the left thigh a few centimeters above the knee up to the groin incision and brought a 4.2 French pediatric Broviac catheter through the tunnel, placing the cuff just inside the insertion site. The insertion site was snugged around the catheter with a 4-0 nylon that was also used to affix the catheter to the leg, and an additional nylon used to further affix the catheter to the leg. An appropriate length of catheter was cut. A small venotomy was made in the saphenous and the catheter was threaded proximally. Fluoroscopy identified the tip lying in the inferior vena cava. I used a 6-0 Prolene suture to secure the catheter in place. The catheter seemed to function very well in terms of infusion and withdrawal, and the femoral vein itself was not compromised in terms of its overall diameter. A 0.25% plain Marcaine block was administered. We noted hemostasis and then closed the groin wound with interrupted 5-0 Vicryl subcuticular sutures. The wound was dressed with a Tegaderm. A Biopatch disc was applied at the catheter skin insertion site. The catheter was curled under a Tegaderm. The entire system was then heparinized with 1.5mL of standard heparin saline flush.
